The "Mi Sheberach" is a public prayer for healing of an individual, said at Temple Beth Am when the Torah is being read on Monday, Thursday, and Shabbat mornings.

If you have a family member or friend who is ill and would like their name mentioned when the Mi Sheberach is recited, please fill out the form below.

Please list the names of those you wish to have the Mi Sheberach said for.  The format for the Hebrew name is person, son/daughter of mother's name.  Below there is room to include three (3) names.  If you have more than three names, please submit an additional form.  Please note: Names will stay on the list for 30 days unless we are notified to remove or retain them. You can update your requests by calling the Temple office at 954-968-4545.
